Has anyone had an LH surge (opk confirmed), and not had the follicle rupture (release the egg)?

I am on clomid and estrogen and have always ovulated on day 14. My cycle is very predictable every month and I have not had cycle problems. I had a positive surge as usual, but my period never came. I had PMS symptoms for 2 weeks!! Pregnancy test was negative, but still no period on day 37. HCG test negative also. My doctor put me on Provera to induce my period. He said I had an LH surge BUT the follicle mustn't have ruptured causing my period to not happen. I was worried more could be wrong with me. This has never happened before and have been TTC for almost 2 years (3 failed pregnancies). Has anyone experienced this and know why? Was it a one time thing or did it continue to happen? Thanks for any help!
